Many people don’t want to move into residential care as they age. If you’d like to stay living independently for longer, in your own home, home care offers a little extra help and support to make it possible. Focus Care can put together a small team focused on your unique needs, then schedule regular visits to support you in your home.
It all depends on your needs. A support worker can help with everything from daily household chores and personal care, to transport and social support. A registered nurse can help with your clinical care needs at home. And our allied health team supports your mental and physical strength. We’re experienced enough to recognise where we could be the most help, and flexible enough to adapt as your needs change.
As you can imagine, the science and research around dementia changes all the time. We practice the Montessori Method – an innovative form of dementia care that focuses on maintaining a connection to the familiar, to help people maintain memory for longer. And that works best in your own home.
A familiar environment is key to effective dementia support. Not only does it support existing memories and routines, but it makes it easier for support workers to identify problem areas, and find ways to manage them. We find care at home the most effective way to offer tailored support.
Focus Care services can be accessed through the government-funded Home Care Package program, or through private payment. Talk to us for help working out which you qualify for, and how best to go about applying for assistance.
A Home Care Package is a government subsidy designed to make it easier for seniors to access home care services, and stay in their homes for longer. These funds can be used to access all of the services we offer. The process to acquire a Home Care Package can be complicated and difficult to navigate. We are here to answer any of your questions and help you through the process. We can also arrange a referral on your behalf for an ACAT assessment.
Applying for a Home Care Package can be complex, but essentially, it’s just 5 steps.
Step 1 — Make sure you’re eligible. You’ll generally need to be aged 65 or over, live in Australia, want to stay living in your home, and need some support to stay independent.
Step 2 — Register with My Aged Care. You’ll need to contact My Aged Care, register, and schedule a home visit with ACAT (Aged Care Assessment Team).
Step 3 – ACAT will assess you. ACAT will visit your home to assess your eligibility and the level of care you require. My Aged Care will then follow up with a letter a few weeks later, telling you whether you’ve been approved for a package.
Step 4 – You will be placed on a waitlist. Once approved for a package, you will be placed on a waitlist known as the National Queue. Depending on the Home Care Package level you have been approved for, your wait for a package to become available can be a few weeks or more.
Step 5 – You are assigned a Home Care Package. You’ll be notified by letter when a Home Care Package is assigned to you. You then have 56 days to choose a provider and activate your package.
Our aim is to match care workers to clients based on their clinical, social, cultural and language needs. We’ll get to know you, your interests and personality type, and will do our best to match you with a support worker who you can form a comfortable bond with. We can’t always guarantee a perfect match, but we welcome your feedback along the way, and always do our best to accommodate your requests.
Care is a universal human characteristic, and we’re proud of the diversity of our team. Our support workers speak over 40 different languages, and while we can’t guarantee there will be one available who shares your cultural background, we’ll always do our best to find someone who understands you.
