They will ensure you find support that meets your needs, no matter how complex. Your support coordinator will also make sure that service agreements and service bookings are completed in an effort to help build your ability to exercise choice and control over your plan.

In essence, a Support Coordinator will assist you to optimise your plan to make sure that you are getting the most out of your federal government funded supports.

Where do I get a Support Coordinator from?

You can look up registered support coordination providers using the Provider Finder tool in the NDIS participant portal myplace. The Provider Finder will enable you to locate support coordination providers that are situated near you. The NDIA publishes a list of registered providers in your state on the NDIS website every three months.

Ongoing Support

Once your support coordination journey is underway, the Focus Care team provides ongoing assistance and guidance. Your current support coordinator will conduct regular reviews to ensure that your NDIS plan remains aligned with your goals.

If you encounter obstacles or changes in circumstances, our support coordination team will work closely with you to find solutions and adapt your plan accordingly.

Finally, our Support coordinators will also help you prepare for NDIS reviews and will advocate on your behalf when necessary to ensure you have access to the level of support you need to live a fulfilling life.

Who can get support coordination funding?

It's the case that not all NDIS participants are provided with funding for support coordination. However, every NDIS participant has the right to submit a request for it. It is advisable that NDIS participants who want to add support coordination to their plan, bring this up at their NDIS planning meetings. The NDIS evaluates each participant on a case-by-case basis.

Support Connection:

  • This is the lowest level of support coordination.

  • It is designed for participants who have lower-level needs and can manage more of their own supports.

  • Support Connection focuses on linking participants with informal, community, and mainstream supports.

Support Coordination:

  • This level of support coordination involves more assistance and coordination than Support Connection.

  • It is suitable for participants with more complex needs.

  • Support coordination helps participants navigate and implement their NDIS plans, manage service delivery tasks, ensuring that all supports are working well together.

Specialist Support Coordination:

  • Support from a specialist support coordinator is the highest level of support coordination.

  • A Specialist Support Coordinator is for participants with with more complex needs who require specialist support.

  • Specialist Support Coordination involves a higher level of expertise and is usually provided by professionals with experience in areas such as health, social work, or complex support needs.

  • The goal of specialist support coordinators is to ensure that services and supports are not only connected but also integrated to achieve consistent delivery of service and the best outcomes for the participant.
