Basic Home Care

Level 1 Package

Basic Home Care Including

Approximately 2 hours of services per week without any contribution or extra supplements
We do not charge Establishment fees
We do not charge Exit fees

Example:

Mr Jones

Avid gardener

Mr Jones is an active and healthy 67-year-old, however he has moderate arthritis in his hips. Mr Jones has a large house and garden and his doctor has recommended avoiding overly strenuous activities. His Level 1 home care package subsidy covers the cost of a Focus Care Worker visiting a couple of hours a week to do the gardening, mow his lawn and change the bed sheets.
If over time Mr Jones’ living conditions change or his arthritis intensifies, Focus Care will help him to be reassessed by ACAT to achieve a Level 2 Home Care Package.

Mrs Panopoulos

Social butterfly

Mrs Panopoulos is in good physical health but has recently been diagnosed with dementia. Her Home Care Package provides her with a dedicated support worker who attends her twice a week to give her daughter a break, help with light housework and take Mrs Panopoulos shopping.
Focus Care matched Mrs Panopoulos with a Greek-speaking support worker to make her feel comfortable and encourage verbal and social engagement.

Intermediate Home Care

Mr McCarthy

Former footy player

Mr. McCarthy is 67 years old and lives alone. As a former professional rugby league player, Mr. McCarthy has a number of long term injuries from his football days, including a cognitive impairment.
Mr. McCarthy uses his Level 3 Home Care Package for weekly physiotherapy, hydrotherapy sessions to improve his mobility and pain management. His support worker also assists him with errands such as getting to and from the shops.
This allows Mr. McCarthy to live an independent life and feel safe and confident that help is at hand.

High Level Home Care

Mrs Trevethan

Passionate cook

Mrs Trevethan is a 92-year old seamstress living home alone with the support of her family. She loves to cook beautiful meals and host dinner parties. Mrs Trevethan has a history of diabetes, heart and liver disease and reduced mobility.
She uses her Level 4 Home Care Package for regular home nursing for two hours per week. Her support worker also visits her for ten hours per week, providing personal care each morning and light domestic duties around the house.
With the help of Focus Care, Mrs Trevethan is able to continue living independently at home and continue doing what she loves most - hosting dinners for her family, cooked with love.
