As a community registered nurse, your role will include:
Assessing needs of clients
Development of support plans
Coordinating allied health appointments
Liaising with third parties (family, service providers, multidisciplinary)
Management of medication in accordance with client needs and legislative requirements
Conducting clinical and person-centred assessments for clients
Providing quality clinical care to clients
Ensuring documentation is in accordance with professional nursing standards and accreditation standards
To be successful for this role, you will need:
Current AHPRA Registration
Evidence of ongoing education
Experience in community care – nursing assessment, clinical care and case management (aged care and rehabilitation)
Current CPR Certification and First Aid Certification
National Police Clearance – less than three years old (or willingness to get one)
Drivers license and own reliable and fully insured vehicle
Willingness to travel to clients in all areas of Sydney
Excellent communication and interpersonal skills
Respect for the rights, dignity and ability of all our clients
Excellent clinical assessment skills and knowledge to assess, diagnose and treat clients
Experience working in the home care/disability field as a nurse
To be a successful candidate for this role, you will also have:
Strong commitment to Community Services and a desire to achieve good outcomes for clients across Aged Care and Disability.
Commitment to learning and professional growth including a willingness to embrace new technologies
Team focus and flexibility to work independently or as part of a team
Initiative and be proactive in your approach in identifying issues and / or solutions
Excellent planning and communication skills
