While your duties will mirror that of a Case Manager, we strongly believe our clients are more than just another “case” for you to handle, and so our case managers are called Care Managers, to better represent their role in coordinating the care that our clients receive.

Our Care Managers take the time to really get to know their clients so that they can most effectively coordinate their care needs. You will help your clients to get the most value out of their Home Care Package, collaborating with them to ensure they’re happy with the quality, cost-effective care they receive. You’ll be there from day one, conducting initial assessments, developing support plans and ensuring each client has what they need, delivered by the right people and services. As time goes on, you’ll assess your clients in case of any changes in needs or circumstances.

Job Description

As a Care Manager your role will include:

  • Completing your clients’ needs assessments and the development of client care/support plans

  • Coordinating with various third parties including, but not limited to: allied health professionals, family, service providers, multidisciplinary

  • Assist in the management of medication in accordance with client needs and legislative requirements

  • Conducting personality assessments for clients

  • Completion of compliant documentation in accordance with accreditation standards

To be successful in this role, you will need:

  • Experience with Home Care Package management

  • Current CPR & First Aid Certification

  • National Police Clearance – less than three years old (or willingness to get one)

  • Drivers license and own reliable and fully insured vehicle

  • Excellent communication and interpersonal skills

  • Respect for the rights, dignity and ability of all our clients

To be a successful candidate for this role, you will also have:

  • Enthusiasm and passion for working with older people

  • Passion in providing excellent coordination of services

  • Commitment to learning and professional growth including a willingness to embrace new technologies

  • Flexibility to work independently and/or as part of a team

  • Initiative and be proactive in identifying issues and/or solutions

  • Excellent planning, communication and interpersonal skills

  • High degree of passion and respect for the dignity and rights of all our clients
